CITATION REQUEST

Any person, business, or organization located in the 13th Norfolk District can request an official Massachusetts House Citation or a congratulatory letter in celebration of a birthday, anniversary, graduation, retirement, special award, special occasion, or achievement. 

Please complete this form and allow at least 2 weeks for our office to process your request. Additional time may sometimes be needed.
